What Is a Lower Body Lift?

A lower body lift — also known as a circumferential abdominoplasty or 360° body lift — is a reconstructive surgical procedure that removes excess skin and tissue from around the entire lower trunk. Unlike a standard tummy tuck that only addresses the front of the abdomen, a lower body lift treats the full circumference: abdomen, flanks, hips, lower back, and buttocks.

The procedure is designed for patients who have significant excess skin following major weight loss, whether through bariatric surgery, lifestyle changes, or natural weight fluctuation. It restores body contour, improves comfort, and addresses functional issues caused by hanging skin.

How Does a Lower Body Lift Work?

A lower body lift is performed under general anaesthesia in a fully accredited hospital setting. The procedure typically involves:

  • A circumferential incision around the lower trunk, usually positioned at or below the waistline so scarring can be concealed beneath clothing
  • Removal of excess skin and soft tissue from the abdomen, flanks, and lower back
  • Abdominal wall plication (tightening of the underlying muscles) to restore core strength and a flatter abdominal profile
  • Buttock elevation and reshaping to restore a more natural contour
  • Waist contouring to improve overall trunk proportions

The incision extends around the full 360 degrees of the body, allowing the surgeon to address all areas in a single operation rather than requiring multiple separate procedures.

Operative Duration

A lower body lift is a major surgical procedure. Operating time typically ranges from 4 to 7 hours depending on the extent of tissue removal and the complexity of the reconstruction.

What Areas Does a Lower Body Lift Address?

The procedure targets multiple areas simultaneously:

Area What's Addressed
Abdomen Excess skin, tissue laxity, muscle separation (diastasis)
Flanks (love handles) Redundant skin folds and tissue overhang
Hips Skin laxity and contour irregularity
Lower back Excess tissue rolls and skin redundancy
Buttocks Sagging, loss of volume and projection
Upper thighs Improvement in the junction between trunk and thigh

Lower Body Lift vs Tummy Tuck — What's the Difference?

A standard abdominoplasty (tummy tuck) addresses only the front of the abdomen. A lower body lift extends the procedure around the entire trunk, making it a more comprehensive operation.

A lower body lift is typically recommended when excess skin extends beyond the front of the abdomen — particularly around the flanks, back, and buttocks. This is common after significant weight loss of 30 kg or more.

Who Is a Good Candidate?

A lower body lift may be appropriate for patients who:

  • Have lost a significant amount of weight and have excess, hanging skin around the lower trunk
  • Are at or near a stable weight and not planning further significant weight loss
  • Are in good general health and fit for a major surgical procedure
  • Do not smoke, or are willing to cease smoking well in advance of surgery
  • Have realistic expectations about the results, recovery, and scarring

Patients who have undergone gastric sleeve, gastric bypass, or other bariatric procedures are among the most common candidates. However, the procedure is also suitable for patients whose weight loss was achieved through diet and exercise alone, or those with skin laxity following pregnancy.

What Are the Risks?

As with any major surgery, a lower body lift carries risks that should be carefully considered. These may include:

  • Wound healing complications, including delayed healing or wound separation
  • Infection
  • Bleeding or haematoma
  • Seroma (fluid collection beneath the skin)
  • Changes in skin sensation
  • Scarring — while scars are permanent, they typically fade over time and are positioned to be concealed by clothing
  • Deep vein thrombosis (DVT) or pulmonary embolism
  • Anaesthetic risks
  • Asymmetry or contour irregularities that may require revision

All risks are discussed in detail during your consultation so you can make a fully informed decision.

Recovery Overview

Recovery from a lower body lift is a gradual process:

  • Week 1–2: Rest at home, limited mobility, surgical drains in place
  • Week 3–4: Light walking encouraged, gradual return to daily activities
  • Week 6–8: Most patients can return to desk-based work
  • Week 12+: Gradual return to exercise and more strenuous activities

Most patients report significant improvement in comfort, clothing fit, and confidence within the first 3 months. Final results continue to refine over 6 to 12 months as swelling resolves and scars mature.

Medicare and Private Health Insurance

In many cases, a lower body lift may qualify for a Medicare rebate under item numbers related to the repair of significant skin redundancy following major weight loss. If you have private health insurance, your hospital stay and some associated costs may also be covered.

Eligibility depends on individual clinical factors. Dr Florica's team can advise on Medicare eligibility and help with insurance pre-approvals during your consultation.
